Information will only be shared with non-medical intermediaries if written consent is given by you. Your contact details will be shared with NHS track and trace if requested during the Covid -19 pandemic.

You are entitled to ask about the information we hold about you and if you wish to exercise your data protection rights this request must be made in writing. You are also entitled to have your personal information deleted, this request will be considered and should also be made in writing but is subject to legal restrictions.
